Documents to be submitted for Internship

Taoyaka Program students who wish to apply/applied for internship are requested to sumit the following documents by the designated date.

How to apply:

Please submit the following documents 3 months before the desired starting date:

  • Application form (Form1)
  • Academic advisor’s Approval (Form2)
  • Internship Implementation Plan (Form3)

Before you do internship…

You are requested to submit the followings after your application is accepted:

  • Detailed description of your internship plan (Form4)
  • Pledge to University (Form5)
  • Pledge to Host Institution (Form6)
  • MOU sample (if needed) (Form7) / Japanese (Form7)
  • Temporary Homebound/Traveling Abroad Report (unique form to each GS)
  • Attachment of Traveling Abroad Report (Form8)
  • Application for financial support (Form9)
  • Receipt of oversea travel insurance and GAKKENBAI

After your internship…

You are requested to submit the followings within 1 weeks after you go back from your internship for getting credits:

  • Certificate of Internship Completion (Form10: issued by the host institution)
  • Internship Report (Form11: written by the student who did the internship)
  • Comments from Academic Advisor (Form12: written by the student’s supervisor)

Financial Support

Travel expense, Accommodation fee and Visa application fee for internship will be provided.
Max 200,000 yen per person (As of 2016)
